Here is the guide to understanding the pivot table mouse cursors:

You have to watch the cursor and also the grey squiggly line. For example, if you drop on the left side of column A, the field will be added to the row area, but will be the outermost field. If you drop on the right side of column A, the field will be the inner field in the row area.
